PhysioLife: Failure to Attend Policy
As our clinics and practitioners often have a waitlist, it is important that if you cannot attend an appointment that you give at least 24hr hours notice. Failure to attend an appointment means that someone else in need was not able to attend in that spot.
We understand that life happens, so if you cannot make an appointment for a legitimate reason, please give our team a call to discuss. Especially if you are unwell on the morning of an appointment, you will not be penalised for being responsible!
Policy:
- Definition of "Failure to Attend": A "failure to attend" is defined as a patient who misses a scheduled appointment without providing at least 24 hours' notice.
- Fee for Failure to Attend: Patients who fail to attend their appointments without sufficient notice will be charged a fee of at least $60. This fee is non-refundable.
Exemptions: The following circumstances may be considered exemptions from the fee:
- Emergency: If the patient is unable to attend due to a medical emergency, documentation may be required to waive the fee.
- Natural Disaster: In the event of a natural disaster or other unforeseen circumstance that makes it impossible for the patient to attend, the fee may be waived.
Payment: The fee for failure to attend will be added to the patient's account and is due upon the next visit. If the fee is not paid, further action may be taken, including but not limited to:
- Denial of future appointments
- Referral to a collection agency
Notice: Patients are encouraged to provide as much notice as possible if they need to reschedule or cancel an appointment. Repeat offenders may be required to pre-pay future appointments.
By scheduling an appointment, patients agree to abide by this policy.
